Proof of Residency: All families will provide proof of residency

  • The verification page from Lee County Assessor
  • A utility bill or cable bill showing the applicantā€™s name, the address listed above and a recent date
  • A signed lease agreement showing applicantā€™s name, the address listed above, and the dates of the current lease (usually this is just the first page of the lease)

Proof of Child’s Age

  • Birth certificate or immunization records are examples of proof of age
    • Alternatives include an adoption record, certified statement of a physician, or anything else that is reasonably reliable.
  • Under Iowa Code section 282.3 a child MUST be five on or before Sept. 15 to enter kindergarten and must be six on or before Sept. 15 to enter first grade.

Medical Documents

  • Doctorā€™s Office copy of your childā€™s Current Immunization Record or Exemption Certificate
    • Immunizations must be on file with the school nurse prior to the child starting school

Initial Registration Form

  • To begin, complete the initial registration form:

  • Once you complete the initial registration form and your request has been approved by office personnel, you will receive an email with instructions on how to login to the PowerSchool Parent Portal and add him/her to your Parent Portal Account.

Student Enrollment Forms

  • Log into PowerSchool to fill out your student forms
    • Once you are logged in, click Forms on the left, then click the Enrollment tab to start completing the Registrations Forms.
    • You must review or complete every section on each form before clicking Submit to move on to the next form.

Iowa Immunization Requirements

The goal of the Iowa Department of Public Health, Immunization Program is to reduce and ultimately eliminate the incidence of vaccine preventable diseases by working in conjunction with public and private health care providers throughout the state. Accomplishing this goal will require achieving and maintaining high vaccination coverage levels, improving vaccination strategies among under vaccinated populations, prompt reporting and thorough investigation of suspected cases and rapid institution of disease control measures.

The Iowa Department of Public Health (IDPH), Bureau of Immunization, has completed the administrative rules process to require meningococcal (A, C, W, Y) vaccine for students enrolling in 7th and 12th grades. The administrative rule change was effective January 11, 2017, and will be implemented at the beginning of the upcoming 2017-2018 school year.
