Health & Registration Requirements

Along with the online registration application and other required forms for enrollment; health documentation also must be provided.

Iowa law requires a parent to verify proper immunization against diphtheria, tetanus, pertussis, polio, measles, mumps, rubella, hepatitis B and varicella to enroll in school.  At least one dose of each immunization must be given before starting school.  An immunization or exemption certificate must be on file with the school nurse by the time the student starts school as this is required by state law.  

Please review the immunization requirements per the Iowa Department of Public Health as the State of Iowa has passed some new requirements in recent years.  The state of Iowa requires students entering 7th grade to have both a Tdap vaccine and a meningitis vaccine.  For those students entering 12th grade, you will need to show proof of two doses of meningitis vaccine.  The meningitis vaccine must be received before the first day of 7th or 12th grade and an updated immunization record must be provided to the school nurse before the first day of school.

Physical Exam
Students entering preschool and kindergarten are required by Iowa law to have a physical examination by a licensed physician and provide proof of exam to the school district.
 
Dental Screening
Required by Iowa law, dental screenings are to be completed for all students entering preschool, kindergarten and ninth grade.  Students entering preschool and/or kindergarten must have had a screening completed no sooner than 3 years of age and no later than four months after school enrollment.  It can be completed by a licensed doctor, advanced registered nurse practitioner, physician assistant, nurse, dental hygienist, or a dentist.  Students entering ninth grade must have proof of a dental screening by a dentist or dental hygienist completed no earlier than one year prior to enrollment and no later than four months after enrollment.  The results of this screening need to be submitted to your school nurse on the designated dental screening form (PDF) which can be located on the Iowa Department of Public Health‘s website.

Vision Screening

The State of Iowa requires students entering preschool, kindergarten, and third grade students to have a vision screening prior to the first day of school.  This exam can be completed by a licensed eye doctor or optometrist, your child’s health care provider at the time of his or her school physical, a free clinic, or a county health department. A screening will be valid if it is done up to 1 year prior to enrollment and no later than 6 months after enrollment.  The results of this screening need to be submitted to your school nurse on the designated vision screening form which can be located on the Iowa Department of Public Health‘s website. Proof of Child’s Age
Birth certificates are the best document for proof of age. Alternatives include an adoption record, certified statement of a physician, or anything else that is reasonably reliable. Under Iowa Code section 282.3 a child MUST be five on or before Sept. 15 to enter kindergarten and must be six on or before Sept. 15 to enter first grade.
 
Proof of Residency: All families will provide proof of residency

The verification page from Lee County Assessor, A utility bill or cable bill showing the applicant’s name, the address listed above and a recent date, A signed lease agreement showing applicant’s name, the address listed above, and the dates of the current lease (usually this is just the first page of the lease)

Log into PowerSchool to fill out your student forms. Once you are logged in, click Forms on the left to start completing the Registrations Forms.You must review, update, and complete each form before clicking Submit to move on to the next form.

Fundraising Form

Keokuk Community School District is fortunate to receive financial support for many activities and initiatives. Please use this fundraising form if you are responsible for leading a school-sponsored activity and/or a school-sponsored organization and wish to host a fundraising event. Once completed, the request will be submitted to the Activities Department. All fundraisers are then submitted to the Board of Education for approval. This form should be completed and submitted by September 13 for all fall and winter fundraising events; by February 14 for all spring fundraising events; and no later than May 15 for any fundraising events that will occur during the summer.
